ACTIVITY INVENTORY DEMO (AI)

This is a computer-assisted interview program for the AI.  The AI is organized into 3 activity Objectives (Daily Living, Social Interaction, and Recreation), 50 activity Goals, and 459 activity Tasks.  First the patient is asked to rate the importance of the Goal on a 4-point scale ranging from "not important" to "very important".  If the patient rates the Goal as "not important", the program moves on to the next Goal, otherwise the patient is asked to rate the difficulty of the Goal on a 5-point scale ranging from "not difficult" to "impossible to do without someone else's help".  If the patient responds "not difficult", the program moves on to the next Goal, otherwise the patient is asked to rate the difficulty of Tasks that are subsidiary to the Goal on the same 5-point difficulty scale.

Thank you for using the LVRN collaborative AI database.  The data you enter for your patients are available for you to use as you please.  You can generate reports for your individual patient's clinical records and you can export your database to spread sheets and statistical packages for analysis.  All identifying information on your patients is protected and available only to you.   By using this system, you agree that a de-identified copy of your database can be merged with other user databases to update the calibration of the AI and obtain normative statistics. None of the data in your database will be accessed for any other purpose without your permission.
